Where can I get tax forms?
You may pick up tax forms at the Central Library and all BPL branches. Because supplies can be depleted quickly, it is wise to call your branch first to check on availability.
At the Central Library, various forms can be obtained at the Social Sciences Department (617.859.2261) on the second floor of the McKim Building (Dartmouth Street entrance). There are also master copies of some forms available for copying; photocopying costs fifteen cents per page.
The basic forms 1040, 1040A, 1040EZ, and the Massachusetts form will also be available in the lobby of the Johnson Building (Boylston Street entrance).
